Medical Consent is for everyone, children for pediatric/vaccine visits or adults coming in for minor illnesses/vaccines.
If getting vaccines, Immtrac Consent is needed. This information submitted to the Texas Immunization Registry, where anyone in the medical field (ex: hospitals, other doctors, etc) have access.
The TVFC form is needed at every visit for children with Medicaid, Chip, or Un-Insured (18 years or younger) when getting vaccines
